Implant shape also plays a role. Round implants can look natural in properly-sized and placed breasts, especially in women with good tissue coverage. Teardrop/anatomical implants are designed to be more natural with more volume in the bottom, and may be suitable.
Implant materials are also an important consideration. Silicone gel implants are common and are considered natural. They are also known as "gummy bear" implants; they feel and move just like natural breasts. Saline (saltwater) implants are safe and effective but may not feel as natural. Modern saline implants are available that simulate gel implants. Implant Placement: Under the Muscle Makes a Difference
The placement of the breast implant in relation to the chest muscle is important for achieving a natural look. Submuscular breast implants (those implants placed behind the pectoralis major muscle) provide greater implant coverage. This helps prevent the implant's edge or rippling from being visible, particularly in women with limited natural breast tissue. The additional coverage helps soften the upper transition of the breast, which improves the "natural feel".

It's tempting to go for a bigger size, which is understandable. But time and again, surgeons tell patients that proportion is key to success.
A general rule of thumb is that the implant diameter should not exceed the natural width of the base of your breast. If an implant is proportioned to the chest, it fits comfortably within the natural base of the breast and looks right at home. Many women who are initially attracted to a larger size discover, following a sizing visit, that a smaller implant gives them the look they desire: full, lifted and proportionate.

For such patients, augmentation and a breast lift (mastopexy) may be combined to regain both size and position. A breast lift with a properly sized implant will look more natural than a large implant in a patient with sagging breasts (or ptosis).
